1. Fire and Life Safety design Plans to be reviewed and approved.
2. Previously approved Fire and Life Safety plans for the existing building.
3. The owner's authorization and NOC to the consultant to proceed with project submission.                                                                          
4. Title Deed (or Proof of Ownership) and Land Map/Plot Plan.
5. Owner's identification/ID.
6. Letter from the owner or company explaining the reason for reapplying/amending.
7.Fire and Life Safety design drawing to be reviewed and approved.

Companies / Government

For consultants:

1. Valid SCDA compliance certificate for the establishment
2. Two (2) registered and trained engineers

For installation and maintenance companies:

1. Valid SCDA compliance certificate for the establishment
2. Valid installation and maintenance license
